What the payroll questionnaire covers

Six short sections – and the reason payroll asks for each of them.

01

Personal details and AHV number

Name, date of birth, nationality, address and marital status, plus your 13-digit Swiss social security (AHV) number starting with 756. Payroll uses them to register you with social insurance and to calculate your deductions correctly.

02

Children

Children entitle you to family allowances, which are paid out together with your salary. If you are taxed at source, they also shape your withholding tax tariff.

03

Bank details

Your bank and IBAN, so your salary is paid straight into your account from the very first month.

04

Employment

Start date, workload, holiday entitlement, wage type and 13th monthly salary – the basis for every payslip. Your employer can complete or confirm this part.

05

Withholding tax

If you are taxed at source, your employer deducts the tax directly from your salary. Your permit, your marital status and, where relevant, your partner’s details determine the right tariff.

06

Additional payroll details

Company car, expense allowances, board and lodging: these items appear on your payslip and on your annual salary certificate.

Who fills in the payroll questionnaire?

The new employee completes the personal sections: personal details, children, bank details, withholding tax and additional payroll details. The employer can complete or confirm the employment section – start date, workload, holiday entitlement, wage type and 13th salary.

What is the AHV number, and where do I find it?

The AHV number is your Swiss social security number: 13 digits, starting with 756. You will find it on your Swiss health insurance card. If you are new to Switzerland, your employer registers you with the compensation office, and you receive your AHV number through that registration.

Who is taxed at source in Switzerland?

As a rule, foreign employees without a settlement permit (C) who live or stay in Switzerland: the employer deducts the tax directly from the salary. Married couples are taxed in the ordinary way instead if one spouse holds Swiss citizenship or a C permit (Art. 83 Federal Act on Direct Federal Taxation). That is why the questionnaire asks for your permit, your marital status and, if applicable, details of your spouse or partner.

Why does payroll need details about my children?

Children entitle you to family allowances (child and education allowances), which your employer pays out with your salary. If you are taxed at source, the number of children also determines your withholding tax tariff.

Can I add my bank details later?

Yes. Hand in the questionnaire with all other details and send your IBAN to your employer as soon as you have it – ideally before the first payroll run.

How are my personal details handled?

The questionnaire goes to your employer and to the payroll team only, and your details are used for payroll and social insurance purposes. Hand it over in person or let your employer upload it to the ALPINUM client portal.

Is there a German version?

Yes: the Personalfragebogen is also available as a fillable PDF and covers the same topics. You will find it together with more templates in our German template library.
